Format of Default Date in Add Helper Should be Consistent

Upon initial entry to the Add helper any date field will show a default ofm/d/yy. If a user selects a date the field updates to m/dd/yyyy (e.g. 3/20/2017) - Note yyyy format On the detail page the date is shown as m/dd/yy
