Add ADPATYP field in the ACCOUNTING SAVE file and NEST_ACTIVITY_TYPE column/label to the PDB tables and CSV files

When trying understand what's going on in your system, you typically need to zoom in at the package level at some point. When doing so, it is important to understand what 'type' of package you are dealing with - if it is just a regular program or whether is related to a stored proc, SQL PL stored proc, UDF, trigger...

When using the OMPE ACCOUNTING reports that the information is available for ACCOUNTING TRACE as well as ACCOUNTING REPORT reports (referred to as 'ACTIVITY TYPE').

When using ACCOUNTING FILE (to produce a load file to load into a Db2 table (PDB), or generate a CSV file) this info is also available as (field) ADPATYP, (column) NEST_ACTIVITY_TYPE .

HOWEVER, this info is NOT available when using ACCOUNTING SAVE (CONVERT) option. As a result , the field ADPATYP and column/label NEST_ACTIVITY_TYPE is not in the PDB table nor the CSV file either
