Improve readahead performance statistic reporting

Since the improved readahead code was established, onstat -p and the associated SMI tables no longer report all readahead pages nor all usage of those pages so that it is impossible to monitor the efficiency of readahead and so be able to determine if more or less readahead is warranted. Additional reporting of RA pages read and RA pages used is required either with new fields/types or by more completely populating the existing fields.
