Prepend machine's name to Assert files written to DUMPDIR

Scenario:  The customer site uses a single network mount for DUMPDIR across hundreds of servers.

Impact:  difficulty in identifying AF files generated by specific servers.

Current workaround:  search online.log for all "stack trace for pid" strings, then search DUMPDIR for the referenced files

Proposed solution:  Prepend the files written to DUMPDIR with a server identifier, e.g. the machine's name

Benefit:  fast, easy identification of all af files (issues) related to a specific server.  This would speed issue investigations, thus leading to faster resolutions.

Users:  DBA teams & IBM/HCL AVP teams

    I think this can be "easily" achieved editing the $INFORMIXDIR/etc/evidence.sh, renaming the file at the end of the script.  

    (I already have to do something similar before in a special situation with memory dumps)

    But an onconfig parameter like AF_PREFIX as an option is better for admin and avoid changes on important scripts.

    Use of <machine name>_<instance name>_af.* would be needed when multiple instanes on the same machine.
