Capture Remote Host & Remote IP for all activities

In the user activity logs. The Remote Host & Remote IP is only captured for the successfull login events. These are not being captured for the failed login attempts and for the logout attempts.

From a security prespective, it is very important that this should be captured for the failed login attempts.

We have had this request from two client.
