Questionnaire Assessment to display object that is associated to question in questionnaire template

Currently you can associate an object (risk or control in my case) to a specific question inside of the questionnaire template (See screenshot 1)

However, when you are answering this question in a questionnaire assessment there is no link/view to see that this object was associated to this question.  (see screen shot 2)

Going one step further, you can add an embedded grid view on this object (in my case risk) to show that it is associated to a questionnaire template, however the available information is really of no value.  (see screen shots 3 and 4)

The Ask:

1.  It would be nice to see the object associated while answering a question, especially if the question is around that record.  That way users do not have to toggle between screens to fill out questionnaire assessments.  

2.  On the embedded grid view or task view of a questionnaire template question, it would be far more useful to have the actual answer to the question instead of meta data around when the question was answered.
