Add a 'Created by' and 'Modified by' to PAW books in a 'Properties' menu

Hi!

We manage an environment that has thousands of users and multiple developers (both internal and external).  The environment is secured in a way where users have access to specific databases and no one should have access to more than one database instance except for environment administrators and some developers.

Occasionally, we'll see find a PAW book created in a folder that should not exist. It would be helpful to know who created this so we can find out the reason, and how it was possible.

This could lead to discoveries of improper security which we'd like to resolve/make sure the users know they should not be doing this.
