Notes and comments for cards and merchants

SP must have the possibility to accept notes/comments on cards and on merchants.
Cards - This functionality must be called by a button click or a link on the card number and must accept several records. This “table” must have 3 field: timestamp, user, notes. Each record can be select in order to delete or change.
Merchants - This functionality must be called by a button click or a link on the merchant number and must accept several records. This “table” must have 4 field: timestamp, user, action and notes. Each record can be select in order to delete or change. The “action” field is a combo box field to select the correct action. These actions can be defined by us and can be update by a csv file.
