IFCID amendment to include SQL Text

Currently the ifcid 366 and 376 do not always supply the SQL text, but rely on the explain statement cache to supply it. This means SMF extracts rarely supply the SQL text required for these records.

Link to source report from report views has been removed. We use lot of report views with set prompts and do change these prompjt values from time to time also point report views to other reports when a newly developed report is to used.
